Gene-Environment Interactions of Adenosine Receptor Gene Family for Metabolic Diseases in Korean Population

Adenosine receptors (ADORAs), encoded by ADORA1, ADORA2A/2B, and ADORA3, are G-protein-coupled receptors that respond to extracellular adenosine. They are known to be involved in various functions in neuronal and cardiac tissues. The activities of ADORAs could be affected by environmental factors such as demographic, physiological, psychological and nutritional factors. It has been reported that their altered activities are associated with chronic diseases such as obesity, diabetes, and cardiovascular disease. The susceptibility in response to environment is different depending on genetic variability of ADORAs. Therefore, we hypothesized that genetic variants in ADORA gene family are associated with the susceptibility to metabolic disease and the associations are modulated by environmental factors. Two studies were conducted to test the hypothesis. In study 1, we examined the effect of coffee intake on prevalence of dyslipidemia depending on genetic variability in ADORAs. We analyzed 3366 females obtained from Korean population based cohort data in Korean Genome and Epidemiology Study (KoGES). Coffee intake was measured with FFQ and classified with Less than one cup (LOC) and Equal or more than one cup (MOC) Although coffee intake was not associated with the incidence of dyslipidemia, genetic variants of ADORA2A, but not other ADORA genes, showed association with coffee intake and the prevalence of dyslipidemia. Subjects carrying minor alleles in most genetic variants of ADORA2A consumed more coffee than those who carrying major alleles. Also, the subset of genetic variants showed increased prevalence of dyslipidemia (rs4822499, odds ratio (OR): 1.609, 95% confidence interval (CI): 1.076 - 2.405, p = 0.0204). However, surprisingly, the prevalence of dyslipidemia in genetic variants was decreased in MOC(rs4822499, OR: 2.194, 95% CI: 1.262 – 3.813, p = 0.0053). In summary, the coffee intake contributes to alleviate the risk of dyslipidemia based on the genotype of ADORA2A. In study 2, we investigate whether genetic variants in ADORAs are involved in the risk of type 2 diabetes according to stress level. We analyzed a cohort study data of 6418 aged over 20 years obtained from KoGES. Stress level was assessed using PWI-SF and the score greater than 27 was considered as high stress group (HSG). HSG showed higher age and BMI and consumed more frequently alcohol than low stress group (LSG). HSG tends to consume less energy, but the higher proportion of carbohydrate in dietary energy intake, compared to LSG. The risk of diabetes is significantly increased in HSG. When we analyzed the associations between ADORA variants and diabetes prevalence depending on stress level, we found that genetic variant of ADORA1 rs6701725 (OR: 1.474, 95% CI: 1.105 – 1.968, p = 0.0084) showed a high risk of diabetes in LSG, but not in HSG. On the other hand, genetic variant of ADORA2B rs2779211 group (OR: 1.346, 95% CI: 1.035 – 1.751, p = 0.0269) and ADORA3 rs2786967 (OR: 1.428, 95% CI: 1.077 – 1.893, p = 0.0134) showed increased risk of diabetes in HSG, but not in LSG. Therefore, the diabetes prevalence was significantly associated with the polymorphisms in some isoforms of ADORA, depending on stress level. In conclusion, the findings demonstrated the effects of environmental factors such as dietary or psychological factors modulated the associations between incidence of metabolic disease and genetic variability of ADORA gene family. Even among individuals carrying the same genotype, they could have different clinical outcomes depending on their own life styles. Thus, we emphasize the understanding of the interaction between genetic variability and environmental factors is crucial for management and prevention of metabolic disease.

아데노신 수용체 (Adenosine receptor, ADORA)는 아데노신을 결합하여 기능한 G단백질 결합 수용체 (G-protein coupled receptor)로서 인체에 ADORA1, ADORA2A, ADORA2B, ADORA3 네 가지 형태로 존재한다. 아데노신 수용체의 기능은 심장 및 혈관, 뇌세포 등 다양한 기관에서 연구되고 있으며, 이들의 기능은 생리적, 심리적, 영양적 요소와 같은 환경 요소에 의해 영향을 받는다. 아데노신 수용체의 비정상적인 기능은 비만, 당뇨 및 심혈관질환과 같은 대사질환과 밀접한 연관성을 보인다. 더욱이 환경인자로 인한 대사질환의 취약성은 유전형에 따라 다양하게 나타난다. 따라서, 본 연구는 대규모 코호트 자료를 바탕으로 아데노신 수용체의 유전적 다형성이 당뇨, 심혈관계 질환과 같은 대사 질환과 어떠한 상관성이 있는지 분석하고 대사 질환 모델에서 환경인자와의 상관성을 조사하고자 한다. 첫번째 연구는 아데노신 수용체의 유전적 다형성에 따라 이상지질혈증의 발병률에 미치는 커피 섭취의 효과에 대해 조사했다. 본 연구는 한국인 유전체 역학 조사 사업 (KoGES)의 안산-안성 코호트의 역학자료 및 유전자 자료를 이용하여 실시되었다. 총 3366 여성 참여자를 대상으로 연구를 진행하였다. 커피 섭취량은 식품섭취빈도조사지를 이용해 조사되었으며 하루 한 잔 이상 커피 섭취자 (MOC), 이하 섭취자 (LOC)를 분류하였다. 결과적으로, 커피 섭취량은 이상지질혈증과 상관성을 보이지 않았다. 하지만 아데노신 수용체 유전자 중 ADORA2A의 유전적 다형성이 있는 참여자는 증가된 커피 섭취량을 보였으며, 이상지질혈증의 위험 역시 높았다 (rs4822499, OR: 1.609, 95% CI: 1.076 - 2.405, p = 0.0204). 더욱 흥미로운 점은 이상지질혈증의 위험이 높은 유전형을 가진 대상자들이 커피 섭취량이 적을 경우 이상지질혈증의 위험이 증가하는 것을 확인할 수 있었다 (rs4822499, OR: 2.194, 95% CI: 1.262 – 3.813, p = 0.0053). 즉, 커피 섭취량은 ADORA2A 유전형에 따라 이상지질혈증의 위험에 영향을 미친다는 것을 알 수 있었다. 두번째 연구는 아데노신 수용체 유전형이 스트레스 정도에 따라 당뇨와 연관성을 보이는지를 조사하였다. 본 연구 역시 한국인 유전체 역학 조사 사업 (KoGES)의 안산-안성 코호트의 역학자료 및 유전자 자료를 이용하여 실시되었다. 총 6418명의 대상자 중 스트레스 자가진단 테스트 (PWI-SF) 점수가 27이상인 경우 고스트레스군 (HSG), 이하인 경우 저스트레스군 (LSG)로 분류되었다. HSG의 경우 LSG에 비해 당뇨의 유병률이 더 높았다. 또한, 아데노신 수용체 유전형을 고려했을 때, ADORA1 rs6701725의 경우 LSG일 때 당뇨의 위험이 높았다 (OR: 1.471, 95% CI: 1.102 – 1.965, p = 0.0088). 반면, ADORA3 rs2786967(OR: 1.443, 95% CI: 1.081 – 1.928, p = 0.0129)의 경우 HSG일 때 당뇨의 위험이 더 높았다. 결론적으로, 당뇨의 유병률은 아데노신 수용체의 다형성과 밀접하게 연관되어 있으며, 그 연관성은 스트레스 정도에 따라 달라짐을 확인 할 수 있었다. 종합하자면, 본 연구는 영양 및 심리적 요소와 같은 환경인자가 대사질환에 미치는 영향이 아데노신 수용체 유전자의 다형성의 영향을 받음을 증명하였다. 우리의 결과는 같은 유전형을 가진 사람일지라도, 그들이 겪은 환경에 따라 질병의 위험이 달라질 수 있다는 것을 제시한다. 따라서 본 연구는 유전적 다형성과 환경인자 사이의 연관성이 대사질환의 예방 및 관리를 위해 중요함을 시사한다.
