Discovery of an inhibitor of ESX-MED23 interaction as a novel strategy to overcome trastuzumab resistance in HER2 overexpressing human gastric cancer

위암은 가장 흔하게 진단되는 암이며, 전세계 암으로 인한 사망률이 2위를 차지하는 질병이다. 위암 환자의 7~34 %가 인간 표피 성장 인자 수용체 2 (HER2)이 과 발현 되어있다는 보고가 있다. HER2는 transmembrane tyrosine kinase (TK) 수용체로서, 암 세포의 증식, 부착, 이동 및 분화와 관련이 있는 것으로 알려져 있다. HER2의 과발현은 예후가 좋지 않기 때문에 현재 HER2는 임상 결과를 향상시키는 중요한 치료의 표적으로 여겨지고 있다. HER2의 인간화 된 재조합 단일 클론 항체로 알려진 트라스투주맙(Trastuzumab)은 일반적으로 HER2가 과 발현 된 암을 치료하는데 사용된다. 이 약물은 HER2 단백질을 특이적으로 표적으로 하여 그 하류 신호 전달 경로를 차단하고 세포증식을 억제한다. 그러나 트라스투주맙으로 치료하고 있는 대다수의 환자는 1년 이내에 약물에 대한 부작용과 내성이 생기게 된다. 이전에 보고 된 트라스투주맙 내성의 분자 기전은 HER2 신호 전달 계통의 비정상적인 활성화, 신호 전달 경로의 보상적 활성화 및 HER2 단백질의 절단이 일어난다. 그래서 전사 조절에서 HER2를 조절하는 것은 트라스투주맙의 한계를 극복하기 위한 합리적인 전략 중 하나가 될 것으로 기대 한다. ETS 전사 인자 계열의 구성원인 Epithelial-specific transcription factor(ESX)는 일반적으로 HER2 단백질 발현의 중요한 조절 인자로써, 알려져 있다. ESX와 MED23 (ESX의 공동 전사 인자)의 상호 작용은 HER2 발현에 필수적이라고 보고되었다. 이 연구에서 우리는 SEAP 리포터 유전자 분석을 수행함으로써 ESX-MED23 상호작용을 효과적으로 막는 새로 합성 된 화합물을 발견하였다. 화합물 중 HT52, 54 그리고 59은 성공적으로 HER2 발현의 감소를 유도하였다. 또한 HT52 와 59는 HER2 mRNA 양과 HER2 양성 위암 세포의 콜로니 형성을 효과적으로 감소시켰다. HT59는 HER2의 하위 신호 전달 경로의 현저한 감소 및 세포 사멸의 유도를 나타냈었다. 우리는 HER2 양성 위암 세포 주 인 NCI-N87을 트라스투주맙의 내성주로 만들었다. 내성 세포주에서는 HER2 단백질이 증가하고 신호 전달 경로가 더 활성화 되었음을 확인하였다. HT59은 트라스투주맙 내성 세포에서 HER2의 발현과 콜로니 형성을 효과적으로 감소시켰다. HT59은 또한 내성 세포주에서 세포 사멸을 효과적으로 유도 하는 것을 보였다. 따라서 HT59은 trastuzumab의 내성을 극복하기 위한 새로운 억제제가 될 것으로 기대된다.

Gastric cancer is the most commonly diagnosed tumor and the third leading cause of the cancer-related death in the world. It is reported that 7~34% of gastric cancer patients overexpress human epidermal growth factor receptor2 (HER2) protein. HER2 is a transmembrane tyrosine kinase (TK) receptor, which is well-known to be related to cancer cell proliferation, adhesion, migration, and differentiation. Since HER2 overexpression is associated with poor prognosis, HER2 is currently regarded as an important therapeutic target to improve the clinical outcome. Trastuzumab, known to a humanized recombinant monoclonal antibody of HER2, is commonly used in the treatment of HER2 overexpressing cancers. This drug specifically targets HER2 protein to block its downstream signaling pathway and inhibit cell proliferation. However, majority of patients experience side effects and resistance to this drug within one year of treatment. Previously reported molecular mechanisms of trastuzumab resistance are aberrant activation of HER2 signaling cascade, compensatory activation of signaling pathway, and truncation of HER2 protein. Regulating HER2 in its transcriptional level is expected to be one of the rational strategies to overcome limitations of trastuzumab. Epithelial-specific transcription factor (ESX), a member of ETS transcription factor family, is commonly known as a key regulator of HER2 expression. It has been reported that interaction of ESX and MED23 (co-transcription factor of ESX) is essential for HER2 expression. By this reason, blocking the interaction between ESX and MED23 has been suggested as a novel strategy to overcome HER2 positive gastric cancer. In this study, we found newly synthesized compounds which effectively interrupted the ESX-MED23 interaction by performing secreted alkaline phosphatase (SEAP) reporter gene assay. Among the compounds, HT59 was selected as a candidate due to its highest efficiency in downregulation of HER2 mRNA and protein expression levels as well as significant inhibition of colony formation in HER2 positive gastric cancer cells. HT59 also showed substantial attenuation of HER2 downstream signaling pathway and induction of apoptosis in NCI-N87 cells. We developed trastuzumab resistant NCI-N87 cells and confirmed that HER2 protein was increased and HER2-mediated signaling pathway was activated in resistant cells. HT59 effectively reduced the expression of HER2 and colony formation in the trastuzumab resistant cells. HT59 also significantly induced apoptosis. Therefore, HT59 would be expected to be a novel inhibitor to overcome trastuzumab resistance
