Purpose: The purpose of this study was to analyze the role of nurses in elderly-centered community integration care and the effectiveness of intervention programs through systematic literature review and to consider foreign community care policies. Methods: Based on databases such as CINAHL, PubMed, Web of Science, ProQuest, and Scoupus, search terms included ‘community integrated care’, ‘aged’, ‘case management’, and ‘nursing role’ between 2010 and 2019. Eleven published articles from 148 screened articles met the inclusion criteria. Results: First, the role of nurse was a care coordinator based on person-centered and integrated care in the primary care setting. Second, the effects of program showed conflicting outcome but there was no clinical significance in the quality of life. Third, developed countries have a policy to help facilitate the link between health care and home services, and national policies are specific so that policies can be realized. Conclusion: This study suggest that education and support should be supported at the national level so that nurses, as care coordinators, can perform an evaluation through understanding and education on a comprehensive assessment tool, draw up care plans with elderly and families, arrange and coordinate services according to the plan, and cooperate with multidisciplinary teams.
