자폐성장애 검사도구에 대한 체계적 문헌고찰

본 연구는 자폐성장애인을 대상으로 하는 국내 검사도구들을 검색하여 체계적으로 분석한 문헌 고찰 연구이다. 본 연구의 목적은 국내에서 발표된 자폐성장애 검사도구 연구들을 정리하고, 이들 평가도구들의 검사체계와 심리측정 및 문화적 특성에 대해 살펴보고자 하였다. 이를 위해 1980년~2021년까지 학술지에 게재된 논문을 검색하여 선정기준에 따라 총 18편의 논문(16개의 도구)을 분석한 결과는 다음과 같다. 첫째, 국내에서 개발된 검사도구가 5개, 국외에서 개발되어 국내에서 타당화가 이루어진 검사도구는 11개였으며, 대부분 조기 선별을 목적으로 하는 검사로서 부모나 교사에 의한 체크리스트 방법이 가장 많이 사용되고 있었으며, DSM-IV 진단체계에서 제시된 것과 유사한 구인으로 구성되어 있었다. 둘째, 본 리뷰에 포함된 연구들은 집단별 평균 점수 차이 또는 민감도, 특이도, 양성예측도 또는 음성예측도, 내적신뢰도를 기반으로 검사도구의 적절한 타당도와 신뢰도를 보고하였다. 셋째, 국외에서 개발된 도구를 국내에서 타당화한 연구 중 5개의 연구에서 국내 참여자를 대상으로 원본과는 다른 절단점을 산출하였고, 국내에서 개발된 도구 중 문화적 특성을 반영한 문항을 직접적으로 보고한 연구는 소수였다. 이상의 연구 결과를 바탕으로 향후 국내 자폐스펙트럼장애 평가도구 개발을 위한 다양한 제안들이 논의되었다.

This literature review study systematically identified and analyzed instruments used to diagnose or screen autism spectrum disorder in South Korea. This review synthesized the studies that developed, standardized, or validated autism assessment instruments for Koreans and examined their psychometric and cultural characteristics. After searching for articles published between 1980 and 2021, we identified 18 studies that met our inclusion criteria. First, five instruments were developed in South Korea, and 11 were developed abroad and validated in South Korea. Constructs of the majority of the instruments resembled those of DSM-IV, and instruments could be applied to a wide age range from young toddlers to adults. Second, studies demonstrated that the majority of the instruments have appropriate validity and reliability by reporting the sensitivity, specificity, positive and negative predictive values, and internal reliability. Third, among the instruments developed abroad and validated in South Korea, five studies newly calculated or examined the cut-off point, and very few examined the cultural characteristics of Korean participants. Based on the findings, we discuss various implications for developing autism assessment instruments in South Korea.
